III. Your Activities

The NST Academic Scholarship Committee would like to know about you and your activities. Please provide answers to the following questions.

IV. Academic Status and Letters of Reference:

Please indicate the name of your current or chosen college or university and your current status. Note that before receiving a check for a scholarship, the applicant chosen to receive a scholarship must be accepted by a degree-granting institution and enrolled in an academic program.

Transcripts:

If you are an entering college freshman, have your current high school transcript sent to the Chair of the NST Academic Scholarship Committee at the address that appears at the bottom of this page. The transcript information should include your cumulative grade point average and your current class rank.

If you have already completed some college work, you should submit a transcript that shows your cumulative grade point average from each college that you have attended.

Two Letters of Reference:

In addition to your transcript(s), you should arrange for two letters of reference, which may be enclosed with your application or may be sent directly to the Chair of the Scholarship Committee.

I certify that the information provided in this application is true to the best of my knowledge. I understand that falsification of information is grounds for disqualification from consideration for an award of the scholarship or for recall of the scholarship money, if already awarded.

Your Chapter President or Designated Representative must sign below. The application will not be considered unless one of them has signed it.
